Correctional Aids Project (CAP)

HIV Education and Discharge Planning for a Healthier Reentry
CAP staff provide HIV education and discharge planning for individuals who are incarcerated and within three months of parole at five participating institutions. Through education, counseling, and coordinated discharge planning, the program works to reduce the risk of HIV transmission while ensuring individuals are connected to the health care, treatment, and support services they need upon release.
By strengthening access to care and community resources, CAP helps individuals transition successfully back into the community while promoting healthier outcomes across New Jersey.
